Stand Down for Sleep A First Responder’s DBT-Informed Guide to Shift Work, Hyperarousal, Nightmares, and Better Recovery

You can finish the call, clear the scene, park the unit, shower, and still feel as if part of your nervous system never got the message that the shift ended.

That is the problem Stand Down for Sleep was written to address.

First responder sleep rarely fails for one neat reason. A night tour can collide with daylight, overtime, caffeine, a rough commute, a replaying call, family schedules, pain, noise, or a body that has become very good at staying alert. Generic advice to “relax” or “practice better sleep hygiene” does not say what to do when your workday ends at sunrise and your brain is still running the scene.

This book gives firefighters, paramedics, EMTs, police officers, corrections professionals, dispatchers, and other shift workers a practical way to sort the problem before throwing more tricks at it.

The first chapters explain why first responder sleep can become fragmented even when you are exhausted. You will learn how sleep pressure, circadian timing, post-call arousal, household demands, and occupational fatigue can overlap. The aim is not to turn you into a sleep scientist. It is to help you identify the part of the problem you can act on and recognize the part that belongs in a sleep clinic, a trauma therapist’s office, or an agency fatigue-management conversation.

For people wrestling with shift work sleep, the book moves past one-size-fits-all rules. Morning light may help one schedule and work against another. A nap can be useful or badly timed. A recovery window may need to be negotiated at home rather than “won” through willpower. The book shows you how to build a plan around the shift you actually work, not the schedule an internet sleep article assumes you have.

The DBT material is equally practical. Stand Down for Sleep uses selected mindfulness, distress-tolerance, emotion-regulation, and interpersonal-effectiveness tools to help with arousal, rumination, difficult emotions, and communication. Readers looking for DBT skills for sleep will find short practices for paced breathing, paired muscle relaxation, temperature change, grounding, defusion, acceptance, opposite action, problem solving, self-soothing, and clear requests at home or work.

These skills are presented with boundaries. They are not sold as comprehensive DBT, trauma treatment, or a guaranteed cure for insomnia. Chronic insomnia has evidence-based treatment, including CBT-I. Suspected sleep apnea deserves medical evaluation. Dangerous sleepiness is a safety problem, not a character test. The book repeatedly shows where self-help ends and professional care begins.

That distinction matters for police officer insomnia, firefighter sleep, and every other safety-sensitive role in which a bad night can follow you into a vehicle, a radio channel, a patient contact, a fireground, a cell block, or a family kitchen. A breathing exercise may help settle arousal. It cannot make severe drowsiness safe for driving. A blackout curtain may protect daytime sleep. It cannot repair an unsafe staffing model. The book keeps those lines visible.

Nighttime replay gets the same treatment. Instead of asking you to relive traumatic events on your own, the chapters on nightmares and hyperarousal focus on present-moment grounding, orientation, low-stimulation coping, and recognizing when recurring nightmares, flashbacks, avoidance, or other trauma symptoms call for licensed care. The goal is to reduce the midnight fight without turning a self-help book into an amateur trauma protocol.

You will also build a repeatable sleep recovery for first responders plan.
