Understanding the MBT Framework
MBT is a structured, evidence-based psychotherapy developed by Bateman and Fonagy, originally for borderline personality disorder and now applied across eating disorders, antisocial personality disorder, adolescent self-harm, and family presentations. Its core focus is the capacity to understand behavior in terms of mental states. This distinguishes it from dialectical behavior therapy, which addresses emotional dysregulation through skill acquisition, and from mindfulness-based approaches, with which MBT shares no theoretical or clinical overlap.

Across thirteen chapters and three appendices, this guide presents the full MBT framework in accessible clinical language: the attachment foundations of mentalizing, the three pre-mentalizing modes, the epistemic trust framework, the collaborative mentalizing formulation, the program structure, the mentalizing stance, and the four-level intervention hierarchy. MBT adaptations for eating disorders, adolescents, antisocial presentations, and families are also covered, along with a comparison of MBT with DBT and schema therapy.
