For much of the twentieth century, the logic of cancer treatment was brutally simple: if cancer was dangerous, treatment had to be equally aggressive.

For women with breast cancer, that often meant radical mastectomy — removal of the breast, chest muscles, and lymph nodes in an operation that could leave lasting pain, disability, and disfigurement. The procedure had become more than a treatment. It was a medical doctrine.

Vera Peters asked whether all of that suffering was actually necessary.

A Canadian oncologist with training in surgery, mathematics, physics, and radiation medicine, Peters built her career by returning to the evidence. She studied patient records, survival tables, treatment histories, recurrences, and long-term outcomes. Her landmark work helped demonstrate that early Hodgkin’s disease could be cured with radiation. Later, she helped show that selected women with breast cancer could achieve comparable survival with breast-conserving surgery followed by radiation instead of automatic radical mastectomy.

Her conclusions challenged some of the strongest assumptions in cancer medicine.

Against the Knife tells the story of a physician who changed treatment not by arguing that cancer was less dangerous, but by proving that effective care did not always require maximum destruction. From operating rooms and radiation departments to hospital archives and consultation rooms, the book follows a quiet battle over evidence, surgical authority, women’s bodies, informed choice, and the meaning of humane medicine.

Vera Peters did not teach doctors to fear the knife. She taught them to ask when it was truly necessary.
